(House, Apartment & Condominium)

• Claim Amount below RM3,000

1. Make a police report
2. Take photo of point of entry and damages
3. Notify Allianz Customer Service at 1300-88-1028 / 03-2264 0700 or UCN at 03-7874 2660 / 012 538 2660
4. Submit the required documents to UCN

• Claim Form – Theft or Burglary Insurance Claim
• Original Police report
• Copy of IC
• Purchase Invoices
• Quotations for repairs/ replacements
• Technician’s Report (If Available)
• E-payment Form

• Claim Amount above RM3,000

1. Make a police report

4. Await Allianz’s adjusters to conduct survey(Note: Do not remove or dispose off the damage items before Allianz’s adjusters have seen the damaged items)
5. Submit the required documents to UCN

4. Submit the required documents to UCN

• Copy of business registration certificate
• Sales / Purchases Invoices
• Stock records
• Business accounts
• Quotations for repairs/ replacements
• Technician’s Report (If Available)
• Business license
• Land titles/tenancy agreements
• E-payment Form
